Abstract
Seven girls and seven boys aged from 11 to 13 years, who were on average 28% overweight, took part in a 3 week weight reduction programme at a diet-camp during which serum CHOL and TG, as well as the lipoprotein fractions (VLDL, LDL, HDL), were determined. The children had a diet of 1000 Cal/day, consisting of 25% protein, 45% fat and 30% carbohydrate. An intensive daily sports programme was also included in the therapy. 50 healthy children of the same age group from a nearby school composed the control group.
The average weight loss after 3 weeks was 4.7±1.12 kg. The total CHOL concentration decreased from 217.5±45 to 167.1±29.5 mg/dl and the TG showed a slight increase from 88.7±21.2 to 98.6±33.8 mg/dl, but this was not significant. The lipoprotein fractions revealed the following changes: The mean CHOL content of VLDL increased from 4.6±4.2 to 10.8±7.4 (P<0.01) and the LDL-Cholesterol decreased from 133.4±41.2 to 75.6±21.8 mg/dl (P<0.001). The HDL-CHOL and TG-content remained constant.
Following therapy the overweight children had lower total CHOL and LDL-CHOL values than the healthy control group (208.9±33 and 111.6±30.8 mg/dl respectively) but these differences were not statistically significant.
The decrease in total CHOL and LDL-CHOL can be accounted for by the following factors:

reduced CHOL synthesis due to negative calorie balance

physical activity

3)
the low CHOL content and higher polyunsaturated/saturated (P/S) fatty acid ratio of the diet.
The substantial change in lipoprotein composition shown after a short time, compared to studies in adults, indicates that in children the lipoproteins are less stable in their composition.
Zusammenfassung
An 14 11–13jährigen Kindern (7 Mädchen und 7 Knaben) mit einem mittleren Übergewicht von 28% wurden vor Beginn und nach Beendigung einer 3wöchigen Reduktionskur außerhalb der Klinik Untersuchungen des Serum-Cholesterins und Triglyceride und der Lipoprotein-fraktionen (VLDL, LDL und HDL) durchgeführt. Während der Therapie erhielten die Kinder einer Diät mit ca. 1000 Cal/Tag, wobei etwa 25% of Protein, 45% auf Fett und 30% auf Kohlenhydrate entfielen; darüber hinaus wurde ein tägliches intensives Sportprogramm abgenhalten. Als Kontrollgruppe wurden 50 gleichaltrige gesunde, normalgewichtige Kinder aus einer Schule in der Nähe von Wien herangezogen.
Die mittleren Gewichtsabnahmen betrugen nach 3 Wochen 4,7±1,12 kg, die Gesamt-Cholesterinkonzentration sank von 217,5±45 auf 167,1±29,5 mg/dl, während die Triglyceride einen leichten, jedoch nicht signifikanten, Anstieg verzeichneten (88,7±21,2 auf 98,6±33,8 mg/dl). Die mit der präparativen Ultrazentrifuge dargestellten Lipoproteinfraktionen zeigten hinsichtlich ihrer Cholesterin- und Triglycerideanteile folgende Veränderungen: Der Cholesteringehalt der VLDL stieg von 4,6±4,2 auf 10,8±7,4 (P<0,01), derjenige der LDL sank von 133,4±41,2 auf 75,6±21,8 (P<0,001) ab. Die HDL blieben in ihrem Cholesterin- und TG-Gehalt konstant.
Nach Ende der Therapie wiesen die übergewichtigen Kinder deutlich niedrigere Gesamt-Cholesterin- und LDL-Cholesterin-Konzentrationen als die gesunde Kontrollgruppe (208,9±33 bzw. 111,6±30,8 mg/dl) auf. Der deutliche Gesamtcholesterin- und LDL-Cholesterin-senkende Effekt wird auf mehrere Faktoren zurückgeführt:

reduzierte Cholesterinsynthese bei negativer Calorienbilanz

körperliche Aktivität
-
3.
geringe Cholesterinzufuhr und hoher P/S-Quotient der Diät.
Da im Vergleich zu Studien bei Erwachsenen derartige wesentliche Veränderungen der Zusammensetzung der Lipoproteine nach relativ kurzer Zeit feststellbar waren, wird eine geringe Stabilität der Lipoproteine hinsichtlich ihrer Zusammensetzung bei Kindern diskutiert.
