Thank you very much for the comment. Dr. Murphy pointed out the importance of post-transfusion purpura (PTP) in the differential diagnosis of ITP. Although the precise pathological mechanism remains unclear, alloantibodies against HPA antigens, especially anti-HPA-1a antibodies made by HPA-1b/1b recipients, appear to play a major role in PTP [
1]. Although HPA-1b/1b individuals are relatively common in the Caucasian population, they are extremely rare in the Japanese population [
2]. Since our manuscript was a translated version of the Japanese guideline, it did not mention PTP [
3]. However, I agree that PTP should be included in the large list of differential diagnosis of thrombocytopenia in the appropriate regions. …
