A 12-year-old boy presented with cough and chest pain. Inspiratory (a) and expiratory (b) posteroanterior chest radiographs taken on the same day show a mass in the lower lobe of the left lung which is more oval in the inspiratory phase (arrows) and more circular in the expiratory phase (arrows). The boy underwent left mini-thoracotomy and intact hydatid cyst was identified. The patient underwent uneventful cystostomy and capitonnage and was discharged. The ovalization of the cyst on inspiration with a more circular appearance on expiration on radiography is known as the “Escudero-Nemerow sign.” It is an important radiologic finding seen in intact pulmonary hydatid cysts.
