A case of neuroborreliosis with striking abnormalities on brain MRI

A 54-year-old male patient, with an unremarkable medical history, presented to our outpatient clinic because of an increase in tremor in both arms. Neurological examination revealed an action tremor in the upper and lower limbs, a mild head tremor and an unsteady gait. Initially, a diagnosis of essential tremor was suspected, however, a brain magnetic resonance imaging (MRI) scan was performed to rule out other causes of tremor. The MRI scan revealed striking symmetric T2/fluid-attenuated inversion recovery (FLAIR) hyperintensities in the pons and the substantia nigra, through the cerebral peduncles and the internal capsule, and flaring out periventricular (Fig. 1). These lesions showed no diffusion restriction or contrast enhancement. In addition, there was an extensive pathological enhancement of the meninges around the brainstem and cerebellum.
