Can radiologists confidently diagnose visceral pleural invasion in small-sized lung cancer?

In the eighth edition of the tumour–node–metastasis (TNM) classification for lung cancer, visceral pleural invasion (VPI) is defined as an independent T2 descriptor, upstaging into T2a all the tumours measuring 3 cm or less, that otherwise should be classified as T1, due to the worst prognosis associated to this finding [1]. According to the seventh and eighth editions of the TNM classification of lung cancer, pathological visceral pleural invasion (pVPI) is precisely defined as “invasion beyond the elastic layer including invasion to the visceral pleural surface” [1]. …
