A 42-year-old woman with a 7-year history of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis initially presented with constitutional symptoms and upper and lower respiratory tract involvement, with radiologic evidence of granulomatous disease affecting the nasosinusal cavities and lung parenchyma. She had previously received induction therapy with glucocorticoids and cyclophosphamide, followed by maintenance treatment with azathioprine and methotrexate, which were gradually tapered after achieving sustained clinical stability. She subsequently remained asymptomatic for 16 months without ongoing immunosuppression. …
