What was the cause of obstructive shock in this patient?

An inferior vena cava filter was deployed in this patient with decompensated liver cirrhosis. However, while receiving invasive mechanical ventilation in the ICU, the patient developed obstructive shock. CT revealed a large hemopericardium with tamponade, but what was the cause?

IBD flare or an infectious complication?

A woman taking adalimumab for ileocolic Crohn’s disease presented with symptoms mimicking a flare. CT revealed extensive venous thrombosis, but respiratory symptoms developed and disseminated tuberculosis was diagnosed.

Etoposide dosing challenges

This case of a testicular cancer patient with cystic fibrosis and liver cirrhosis shows that monitoring of etoposide concentrations can be beneficial in complex scenarios involving organ dysfunction and/or potential drug–drug interactions.

K. pneumoniae-induced liver abscess in ulcerative colitis patient

This unusual case reports a hepatic abscess secondary to K. pneumoniae infection in a woman with UC. Culture-directed antibiotics were given alongside treatment optimized specifically for the patient's underlying UC.

ICI-associated gastritis in an RCC patient

A case of immune-related gastritis complicated by subsequent cytomegalovirus gastritis in a patient undergoing combination therapy with ipilimumab and nivolumab for renal cell carcinoma.

Esophageal bezoar caused by enteral nutrition

Dysphagia and nasogastric tube obstruction occurred after 5 days of enteral feeding following cardiothoracic surgery. Upper endoscopy revealed a large, whitish bezoar completely filling the esophageal lumen.

Localized bile duct stricture mimicking cholangiocarcinoma

A case of eosinophilic cholangitis presenting as a localized biliary stricture in a 65-year-old man. As perihilar cholangiocarcinoma could not be ruled out, the patient opted for surgical intervention.

Pancreatic cancer or isolated pancreatic sarcoidosis?

Acute pancreatitis was diagnosed in a woman who presented with abdominal pain. Imaging raised suspicion for obstructive pancreatitis secondary to pancreatic cancer, but later investigations revealed a more unusual cause.

Duodenal biopsy gone bad

Hematoma, obstruction, and hemoperitoneum in a 25-year-old man admitted to the emergency department for mild abdominal pain and vomiting episodes 2 days after EGD with biopsy for suspected celiac disease.

Long-term response to nivolumab in gastric cancer

Adjuvant treatment for metastatic melanoma with six cycles of nivolumab also led to a sustained response of the concurrent early-stage gastric cancer in this patient, who achieved long-term disease-free survival with minimal treatment.

Acute hepatitis secondary to gonococcal infection?

Acute hepatitis and Neisseria gonorrhoeae bacteremia were diagnosed in a patient presenting with abdominal pain. Treatment of the disseminated gonococcal infection greatly improved abdominal symptoms and hepatic injury.

Primary leiomyosarcoma of the colon

Colonic leiomyosarcoma is a rare and aggressive tumor that is often difficult to diagnose preoperatively, especially in a case such as this where multiple biopsies were inconclusive.

Long-term remission of ulcerative colitis-associated scleritis after proctocolectomy

A case of severe, intractable scleritis in a man with refractory ulcerative colitis. Scleritis resolved immediately after proctocolectomy, suggesting intestinal–ocular interaction in the pathogenesis of ulcerative colitis.

Ulcerative colitis complicated by C. difficile infection

Screening for C. difficile infection should be considered in ulcerative colitis patients with worsening symptoms. In this case, there was a partial response to oral vancomycin and guselkumab was initiated alongside mesalazine.

Pediatric CRC: a rare diagnosis

Colorectal cancer is very rare in children and adolescents. And due to the generally highly aggressive nature and a low index of suspicion in this population, diagnosis can be delayed and lead to a very poor prognosis.

Perforation during treatment of refractory duodenal ulcer bleeding

Perforation is rare when using over-the-scope clips. In this case, clip detachment is thought to have damaged the ulcer wall while deployment of a second clip caused transmural injury, as revealed by laparotomy.

Rare complication after giant hiatal hernia repair

A 64-year-old woman underwent laparoscopic repair of a giant paraesophageal hernia but developed symptoms of complete esophageal obstruction due to food impacted in the distal esophagus. What was the cause?

Laparoscopic retrieval of a retained intrahepatic metal nail

A 21-year-old man presented to the emergency department after sustaining a nail gun injury to his epigastrium. The metal nail was found to be embedded in the left lobe of the liver and was successfully retrieved laparoscopically. 

Sigmoid volvulus in a healthy young woman

A high index of suspicion for sigmoid volvulus is needed in young women with abdominal symptoms as the overlap of gastrointestinal and gynecological symptoms can easily lead to misdiagnosis.

Enteritis and peritonitis as first presentation of SLE

Another case report contributing to the growing body of literature on gastrointestinal involvement in systemic lupus erythematosus, and underscoring the importance of early recognition and targeted treatment.
