In this issue, Powell and colleagues [
1] evaluate implementation of a care bundle to standardize management of obstetric anal sphincter injury (OASI), focusing on three objectives:
1.
Second-generation cephalosporin administration at repair
2.
Wound check within 2 weeks postpartum
3.
Repair in the operating room (OR)
Clinician education coupled with an OASI order set in the electronic health record accomplished the first two goals, but OASI repair in the OR remained uncommon post-implementation. The authors suggest myriad possible explanations, all of which undoubtedly contribute, but the largest reason is that this objective requires behavior change on the part of providers and patients: a much heavier lift than can be accomplished through education and an electronic order set. …
