Intrahepatic cholangiocarcinoma (IHCC) is a rare but aggressive biliary tract cancer (BTC) with distinct anatomic, molecular, and clinical characteristics that distinguishes it from other BTCs. For patients with localized IHCC, surgical resection is the mainstay of treatment and involves performing a margin-negative resection and portal lymphadenectomy. This approach is associated with improved long-term survival compared with individuals who do not undergo surgical resection, yet recurrence following curative-intent resection remains high. In fact, patients who undergo surgical resection for IHCC have nearly a 25% incidence of recurrence after only 6 months.1 As such, novel strategies to improve the outcomes of patients with potentially resectable disease are desperately needed. We therefore read with great interest the study by Maithel and colleagues regarding the feasibility and safety of neoadjuvant combination gemcitabine, cisplatin, and nab-paclitaxel (GAP) among patients with resectable, high-risk IHCC. …
