Perihilar cholangiocarcinoma often presents a poor prognosis, with surgical resection being the primary curative option. Emerging evidence suggests that laparoscopic surgery may offer promising outcomes. This case report details a successful laparoscopic approach for Klatskin tumor type IIIb. A 49-year-old male presented with epigastric pain. Initial laboratory results showed normal levels of CA19-9, bilirubin. Imaging confirmed perihilar cholangiocarcinoma type IIIb. A laparoscopic procedure was performed. The operation lasted 440 min, blood loss 300 ml with no transfusion, and R0 resection margin was achieved. The patient was discharged after 11 days without complications. Laparoscopic surgery is feasible to get radical resection for perihilar cholangiocarcinoma. Further studies are needed to evaluate long-term outcomes and refine its indications.
