A rare case of a young patient with an aortic sinus aneurysm invading the ventricular septum and dissection caused by Behcet’s disease. He presented with recurrent chest tightness and shortness of breath.

Patient with longstanding history of repaired aortic rupture and replacement of endograft presented with new aorto-bronchial fistula requiring extra-anatomic aortic bypass and radical debridement of contaminated tissue.

Rete mirabile usually occurs in the middle cerebral or internal carotid artery. This is the first report of unilateral rete mirabile in multiple intracranial arteries with ipsilateral internal carotid artery agenesis.
