Primary rectal cancer staging is performed using MRI and is underpinned by the use of multiple high-resolution two-dimensional small field of view (SFOV) turbo spin echo (TSE) T2-weighted (T2w) acquisitions [1, 2], each typically taking around 5 min to complete. Accurate T-staging requires detailed visualization of the relationship between tumor and rectal wall layers and is essential for directing management. For example, T1 tumors may be suitable for local excision, reducing morbidity from transabdominal resection whilst realising acceptable oncological outcomes, and tumors confined to the muscularis propria (T2) or with early extension beyond (up to T3b) without nodal involvement or other high risk features may be suitable for a straight-to-surgery approach sparing potential morbidity from neo-adjuvant therapy [3, 4]. …
