We commend Richard et al. [1] for their pivotal trial comparing pressure-controlled ventilation with spontaneous breathing (PC-SV) to volume assist-control ventilation (ACV) in acute respiratory distress syndrome (ARDS) patients. The study highlights PC-SV’s potential in reducing sedation and minimizing neuromuscular blocking agents, while lowering the need for adjunctive hypoxemia treatments. However, the balance between lung protection and risks such as patient self-inflicted lung injury (P-SILI) and diaphragmatic injury requires further investigation, particularly in patients with heterogeneous lung injury. …
